Welcome back to the new school year and a special welcome to our incoming 6th grade families from the Blake Middle School Nurses, Tricia Williams and Meghan Carlson.  We are looking forward to meeting you and your students!

Please review important health and medication guidelines listed below.

Medication Guidelines & Drop-Off

  • Required Documentation: If your student needs medication at school, you must submit a Parent/Guardian Medication Administration Plan along with a current medication order from their healthcare provider. These forms must be completed for all medication to be administered at school including daily medications as well as emergency medications. Students may not carry nor self administer any medication at school or on school field trips without both forms on file in the nurse’s office for the current school year.

  • For students with an epinephrine and/or inhaler order, please contact your student’s health care provider to request an updated Allergy Plan and/or Asthma Plan which serves as the required medication order. 

  • Packaging: All medications must arrive in their original pharmacy bottle or box with the original pharmacy label attached.  Please remember to check the medication expiration date.

  • Drop-Off: Please plan to stop by with your student’s medication and above noted medication forms on the morning of the first day of school.  You are also welcome to drop off medication to the Blake School Nurse’s office on Tuesday, September 1, 2026 between 8:45am - 10:45am when a nurse will be available. 

Illness Policy & Health Updates

  • Students with a fever of 100.4°F or higher must remain home until they are fever free for 24 hours without the use of fever-reducing medications (such as Tylenol or Advil/Motrin/ibuprofen). Please see our Illness Guidelines for more details.

  • Please call the school absence line at 508-242-8501 each day your child will be absent or late for school or send an email to blakeattendance@email.medfield.net  In addition, please send an email to blakeattendance@email.medfield.net if your student is being dismissed early from school.

  • Medical Updates: Please inform the school nurses of any new health changes, significant illnesses, injuries, or hospitalizations by calling us at 508-242-8554. We will collaborate with your student’s teachers and school counselor to ensure any necessary accommodations are in place.  

Upcoming Health Screenings

The Blake Middle School nurses conduct the following health screenings as required by the MA Department of Public Health during the school year.

  • 6th Grade: Postural Screening

  • 7th Grade: Hearing & Vision, BMI (Height/Weight), Postural and SBIRT Screenings

  • 8th Grade:  Postural Screening

We send email notifications to parents/guardians approximately two to four weeks before screenings begin which include instructions on how to opt out your student(s) from the screenings if you prefer. 

​Physicals  

Massachusetts State Law requires that all students entering 7th grade have a current physical on file in their school health record. In preparation for 7th grade, we ask 6th grade parents/guardians to email us or send in a copy of your student’s 6th grade physical whenever their physical is done from June 1, 2026 - May 31, 2027 in order for us to be compliant when students enter 7th grade.  

A current physical is also required for all students who play interscholastic (school) sports. Physicals are valid for 13 months from the date of the physical.
