Health Forms due back by April 2, 2026
Health forms for the 8th grade trip to Washington, DC were sent home with students on March 4, 2026. Please read through the Overnight Field Trip Health and Medication Guidelines included with the forms.
Completed health forms are due back in school by Thursday, April 2, 2026. Your 8th grader can hand the forms to their advisory teacher, Mrs. Avery (counseling office), health office or main office. Click here if you would like an extra copy of the health forms packet.
