Dear 8th grade Parents/Guardians:
Welcome back to the new school year. We hope you and your family had a healthy and enjoyable summer. Please reach out to us in the middle school health office if your student developed any health issues over the summer that we should be aware of including a new health diagnosis, change in medication, significant injury, illness and/or hospitalization.
We would like to pass along a few reminders to keep in mind as we start off the new 2025-26 school year:
Students are
not
allowed to carry nor self-administer any medications at school including prescription and non-prescription medications with the exception of emergency medications (Epinephrine for life-threatening allergic reactions and inhalers for asthma) which require an updated Allergy and/or Asthma Plan.
If your student needs medication while at school, please fill out this
which requires both your signature as well as the signature of your student’s health care provider.
All medication must be in a pharmacy issued container with a prescription label attached or the original manufacturer’s packaging labeled with your student’s name and be accompanied by a current medication authorization form as noted above signed by you and your student's health care provider.
Prescriptions for controlled medications must be hand delivered to the school nurse by a parent/guardian.
Please note - A completed medication authorization form is not needed for Tylenol and Advil/Motrin/ibuprofen which the school nurses can administer in the health office with your permission.
We will be available in the middle school health office from 12:30pm - 2pm on Tuesday, August 26, 2025 if you would like to stop by and drop off your student’s medication, including Epinephrine and inhalers, along with completed medication orders.
Please review our Illness Guidelines for guidance on when to keep your student home from school if they are feeling ill. If your student will be absent or tardy to school, please notify the main office at the middle school by calling the absence line at 508-242-8501 or send an email to blakeattendance@email.medfield.net
