STUDENT MILITARY OPT-OUT FORM

In accordance with the No Child Left Behind Act (NCLB) and the National Defense Authorization Act (NDAA), school districts must now provide, upon request, the names, addresses, and telephone numbers of all secondary students (grades 9-12) to military recruiters and institutions of higher education provided that the school notifies parents and students of their right to request that this information not be released without their prior written consent and that the school complies with such requests.

Parents and students who are at least fourteen years old may request that the student's name, address, and telephone number not be released.  Complete this FORM if you are requesting that your student's name, address, and telephone numbers(s) NOT be released to Armed Forces, Military Recruiters, or Military Schools.

This FORM must be submitted by October 1, 2025.

FROM THE MHS PTO

Did you know? For years, Medfield parents and Guardians have supported our school by paying PTO dues. Back in the day, an envelope would come home, sit on the kitchen counter, and eventually find its way back with a check inside.

Now that everything is paperless, many families forget — and our contributions have dropped. But our students, teachers, and school still need us more than ever!

Where your dues go:

    •    Funding grants for classroom and extracurricular needs

    •    Scholarships for worthy students

    •    Inviting inspiring guest speakers for us all to learn from

    •    Teacher appreciation lunches/snacks throughout the year

    •    Supporting School Spirit events

Your dues help keep Medfield High School an extraordinary place for learning, growth, and community.

(Suggested amount $50 per student)

It’s EASY to contribute today:

    •    Online - www.medfieldhspto.com

    •    By check - Payable to MHS PTO, Medfield High School, 88R South Street, Medfield, MA 02052

    •    Or Pay by Venmo - @Medfield-HSPTO

Every contribution — large or small — adds up to make a lasting impact. Let’s show our students and teachers that we’re behind them 100%.

💙 Let’s continue the tradition.

FROM THE HEALTH OFFICE

Welcome back!

Please look for an email from us in the next week for the link to your student's Emergency Health Form from last year.  Be sure to review and submit them promptly.   If your student is new to Medfield Public Schools we have already received the paper form you filled out at registration.

  • If your student has an epipen, please send us the signed (by prescriber and parent) Allergy Action Plan and indicate whether they have their epipen in their backpack or we will hold it in the nurse's office. A signed Allergy Action Plan directly from your students provider is also acceptable.  Epipens must also be in their original pharmacy box with a pharmacy label attached.

  • If your student will need medication while at school, please fill out the Medication Authorization Form. Students can not carry it in their backpack without this form being filled out.

Always let us know if there is any medical information that changes such as a significant illness, injury, hospitalization etc. Always let us know if there is any medical information that changes such as a significant illness, injury, hospitalization etc. We will communicate with your student's teachers to be sure they get any accommodations that are needed.

For all illnesses, your student should remain at home until 24 hours without a fever without fever reducing medications such as tylenol and advil. Here are our Illness Guidelines.

The above forms, and other useful information, can be found at the MPS Homepage Health Forms and Health Information.

TURNING 18: WHAT YOU NEED TO KNOW

Before the Open House on September 25, from 6 - 6:30pm in the HS LMC, Attorney Deb Hedges will discuss the legal implications of turning 18 in Massachusetts. A key consideration is the set of legal documents needed to allow parents or guardians to continue assisting with financial, legal, and health decisions for their adult children after they come of age. She’ll provide blank forms for these documents - HIPAA, Power of Attorney, Health Care Proxy, and FERPA - so students will be informed and ready when the time comes to sign them after their 18th birthday. Her law firm, Borchers Trust Law, will also offer a free Document Signing Day in the spring.

Welcome back! For details regarding the MHS yearbook, please read through this 2026 Yearbook FAQs doc. One item of note is how to purchase a copy ahead of distribution in June. Click on https://jostensyearbooks.com/?REF=A01095588 to order a copy...the earlier, the better. The cost is $80 now and goes up incrementally to $110 at the end of the school year. Either way, it is a steal! Our prices haven’t gone up in years despite rising production costs. Thanks in advance for your support!
