Dear Medfield Families,
Our next vaccination clinic is scheduled for Wednesday, November 17th at the Dale Street School from 3:30 PM to 6:30 PM. We will be partnering with Fallon Ambulance Company for this and one other clinic. You can sign up here Please bring a copy of your insurance card if you have insurance. There is no cost for the vaccine.
For the students vaccinated at yesterday's clinic only: On Tuesday, November 30th, a clinic will be held for second doses for those students who attended the clinic at Dale Street School on November 9th. Please bring your vaccination card and complete: COVID-19 Vaccine Consent Form for Individuals 5-17 Years of Age and if you have insurance, bring a copy of your insurance card. Please sign up here for your appointment for your second dose.
