Kindergarten Registration

Dear Parents and Guardians,

Welcome to the Registration Page for Kindergarten. Your child is eligible to attend kindergarten if he/she is five years old on or by August 31, 2013. This is a very exciting time for children, family members, and my staff. We look forward to establishing a positive relationship with you and each student. Please follow the directions below to complete the registration forms.

After you register, we recommend that you check this page for updated information. We will inform you directly of events, but checking this site will give you advanced notice of dates and times.

How to Register

To register a student to start Kindergarten in the Fall of 2013, print and complete the forms listed below then return to the Memorial School at 59 Adams Street. All required forms must be received in the school office by February 15, 2013 for your child to be eligible for the AM and Full Day Kindergarten Lottery. Session assignments will be completed by mid-April and you will be notified by mail. If you have difficulty obtaining any of these forms, please contact the Memorial School Office at (508) 359-5135 between the hours of 8:00 a.m. and 3:30 p.m.

State Mandated Health Requirements

A physical exam, which includes a vision screening, and immunization history are required for entrance to kindergarten. Your child can be excluded from school until we have received documentation from your physician. Please submit a copy of the following information to Memorial School no later than June 1, 2013.

Immunization Record -The law requires that the following immunizations and/or screenings be complete before entering kindergarten.

Hepatitis B: 3 doses
DPT: 5 doses
Polio: 4 doses
MMR: 2 doses
Varicella: 2 doses
Lead Screening: typically done in preschool, please provide screening date

Vision Screening - Massachusetts law states that upon entering kindergarten or within 30 days of the start of school, the parent or guardian of each child shall present certification that the child has passed a vision screening within the past 12 months. In years past, vision screening was offered at the school during the spring prior to the kindergarten year. It is now required that a primary care provider perform the screening.
